Food Plan Review Application
A plan review application may be obtained from Tacoma-Pierce County Health Department or by calling 253 798-6460, Monday through Friday, 8:00 a.m. to 4:30 p.m or you may download an application from this page.
Plan Review Application and Checklist: Application for construction of a new permanent facility.
Mobile Truck/Trailer Plan Review Application and Checklist: Application for mobile facilities.
Mobile Cart Plan Review Application and Checklist: Application for mobile carts such as an espresso or hot dog stand.
Remodel, Change of Menu and Equipment, WSDA Exempt Wholesalers, and Commissary Relocation Application: Application for facilities planning to remodel or make a change to an existing menu or equipment.
All applications which do not have the required information will be returned to the applicant for completion before being reviewed. A resubmittal fee will also be charged for any applications returned due to lack of requested information. Remember, submission of a complete plan review application will allow for a faster approval.
